Leadership & Management
Our leadership team brings extensive experience across disability support, service coordination, and community care. They set the standards for quality, accountability, and person-centred practice across every part of Elevate. Actively involved in service delivery, they ensure strong oversight, clear communication, and a culture of care, professionalism, and trust.
Care Coordination Team
Our care coordinators are central to seamless support. They oversee services, monitor progress, and maintain consistent communication with participants and families. Collaborating closely with support workers they ensure services remain aligned with individual goals. Professional and clinical expertise informs their work, giving participants and families confidence in the safety, quality, and effectiveness of every support.
Occupational Therapy Team
Our occupational therapists provide specialist, goal-focused programs that build functional skills, independence, and confidence. Each program is tailored to the individual, supporting meaningful participation in daily life. Services are delivered with a strong focus on collaboration, practical outcomes, and respect for each person’s pace and aspirations.
Support Workers
Our support workers are the heart of everyday support. They assist with personal care, daily routines, domestic tasks, mentoring, transport, and community participation, always with patience, professionalism, and respect. Administrative Team
