Once your initial plan is in place and the transition period is complete, our care coordinators take the reins, overseeing the delivery of every support. They ensure services remain consistent, goal-focused, and responsive to any changes. They’re the steady presence who check in regularly, liaise with families and participants, and make sure every interaction is meaningful, empowering, and aligned with your personal objectives.
Our support workers and occupational therapists work hand-in-hand with coordinators to provide day-to-day assistance – from personal care and mentoring to community participation, domestic support, and skill development.
